  • Name           
  • Gender
  • Email address
  • Phone number
  • Valid shipping address
  • T-shirt/Sweatshirt size
  • Transcript/Letter of acceptance
  • Resume
  • Name of Canadian University
  • Department/Program
  • Year of study
  • If they have applied to a BAI internship or received a fellowship
  • Skills or experience related to computer science
